Son incidence ne cesse d&rsquo;augmenter chez les femmes et l\u2019\u00e9cart de l\u2019incidence entre les deux sexes diminue au niveau mondial. L&rsquo;objectif de cette \u00e9tude est d&rsquo;\u00e9valuer le profil \u00e9pid\u00e9miologique, clinique, histologique, mol\u00e9culaire, th\u00e9rapeutique et le pronostic du cancer du poumon non \u00e0 petites cellules (CPNPC) chez la femme.\n          <br><br>\n          <strong>M\u00e9thodes :<\/strong> C\u2019est une \u00e9tude r\u00e9trospective incluant des patientes atteintes du CPNPC confirm\u00e9 et suivies au service d\u2019Oncologie M\u00e9dicale du Centre Hospitalier Universitaire Mohammed VI de Tanger entre mai 2018 et d\u00e9cembre 2023.\n          <br><br>\n          <strong>R\u00e9sultats :<\/strong> 77 patientes ont \u00e9t\u00e9 incluses dans l&rsquo;\u00e9tude. Leur \u00e2ge moyen \u00e9tait de 60.5 ans. Toutes les patientes \u00e9taient non fumeuses. Le d\u00e9lai moyen du diagnostic \u00e9tait de 5.1 mois. Le suivi m\u00e9dian \u00e9tait de 5 mois. Les signes cliniques majeurs \u00e9taient la dyspn\u00e9e (42.8%), la toux (31.1%) et la douleur thoracique (25.9%). La majorit\u00e9 des patientes \u00e9taient diagnostiqu\u00e9es au stade IV (90.9%).\nL&rsquo;ad\u00e9nocarcinome \u00e9tait le type histologique le plus fr\u00e9quemment diagnostiqu\u00e9 (87%). Les localisations m\u00e9tastatiques les plus fr\u00e9quemment retrouv\u00e9es \u00e9taient la pl\u00e8vre (55.7 %), les os (44.3 %) et le poumon (30%). Le test de mutations du g\u00e8ne de l\u2019EGFR r\u00e9alis\u00e9 chez 27 patientes \u00e9tait positif chez 13 patientes (48.1%). Les mutations retrouv\u00e9es \u00e9taient exclusivement celles au niveau de l&rsquo;exon 19 et la mutation ponctuelle L858R dans l&rsquo;exon 21 qui sont couramment associ\u00e9es \u00e0 la sensibilit\u00e9 aux inhibiteurs de la tyrosine kinase (ITK) de l&rsquo;EGFR. La mutation ALK qui a \u00e9t\u00e9 recherch\u00e9e chez 8 patientes \u00e9tait positive dans 12.5% des cas. La chimioth\u00e9rapie palliative de premi\u00e8re ligne a \u00e9t\u00e9 indiqu\u00e9e chez 54 patientes dont 46 avaient re\u00e7u une chimioth\u00e9rapie \u00e0 base de sels de platine (5 d\u2019entre elles \u00e9taient EGFR mut\u00e9)\net 8 EGFR mut\u00e9 avaient b\u00e9n\u00e9fici\u00e9 d\u2019une th\u00e9rapie par les ITK de l\u2019EGFR (erlotinib ou gefitinib). La survie globale (SG) m\u00e9diane des patientes mut\u00e9es pour l\u2019EGFR trait\u00e9es en premi\u00e8re ligne par les ITK de l\u2019EGFR \u00e9tait significativement plus longue par rapport \u00e0 celle des patientes EGFR mut\u00e9 trait\u00e9es par chimioth\u00e9rapie \u00e0 base de sels de platine (20 mois contre 9 mois, p=0.05) et la survie sans progression (SSP) m\u00e9diane chez ces patientes \u00e9tait \u00e9galement significativement plus longue par rapport \u00e0 celle des patientes trait\u00e9es par chimioth\u00e9rapie \u00e0 base de sels de platine (19 mois contre 5 mois, p=0.028).\n          <br><br>\n          <strong>Conclusion :<\/strong> La population des femmes atteintes de CPNPC \u00e9tudi\u00e9e \u00e9tait caract\u00e9ris\u00e9e par l\u2019absence de tabagisme, la dominance du type histologique ad\u00e9nocarcinome et par un pourcentage \u00e9lev\u00e9 de mutations\nde l\u2019EGFR. La SSP et la SG m\u00e9dianes des patientes trait\u00e9es par les ITK de l\u2019EGFR \u00e9taient\nsignificativement plus longues que la SSP et la SG des patientes qui avaient re\u00e7u une chimioth\u00e9rapie \u00e0 base de sels de platine.
